Annual Family Income Survey-Encuesta de ingresos anual

Parent/Guardians of Highland School District Students,
All students in the Highland School District receive free breakfast and lunch. However, we still need families to complete the annual Child Nutrition Eligibility & Education Benefit Survey/Application. The information collected helps determine the percentage of students who qualify for free or reduced-price meals, which is used to calculate the amount of funding our district receives for the Learning Assistance Program (LAP). This information will also help determine whether your family qualifies for the Summer EBT program.
We have sent home a green survey form for families to complete. If you have more than one student in your household, you only need to complete one survey and include all members of your household on that form.
Online Option
This year, families also have the option to complete the survey online through Skyward Family Access under the Food Service tab.
To complete the application online:

  1. Log in to Skyward Family Access.
  2. Click the Food Service tab.
  3. Click the Applications button near the top of the screen.
  4. Select “Add Application.”
  5. Complete and submit the application.

Please note: If you have multiple students, you cannot submit an application while in the “All Students” view. You must select an individual student before adding an application. However, the application will apply to all members of your household, so you only need to submit one application per household.

A los padres o tutores de los estudiantes del Distrito Escolar de Highland:
Todos los estudiantes del Distrito Escolar de Highland reciben desayuno y almuerzo gratuitos. Sin embargo, necesitamos que las familias completen la encuesta/solicitud anual de elegibilidad para nutrición infantil y beneficios educativos. La información recopilada ayuda a determinar el porcentaje de estudiantes que califican para comidas gratuitas o a precio reducido; este dato se utiliza para calcular la financiación que recibe nuestro distrito para el Programa de Asistencia para el Aprendizaje (LAP, por sus siglas en inglés). Esta información también ayudará a determinar si su familia califica para el programa Summer EBT.
Hemos enviado a casa un formulario verde de encuesta para que las familias lo completen. Si tiene más de un estudiante en su hogar, solo necesita completar una encuesta e incluir a todos los miembros de su hogar en dicho formulario.
Opción en línea
Este año, las familias también tienen la opción de completar la encuesta en línea a través de Skyward Family Access, en la pestaña "Food Service" (Servicios de alimentación).
Para completar la solicitud en línea:
Inicie sesión en Skyward Family Access.
   1. Haga clic en la pestaña "Food Service".
   2. Haga clic en el botón "Applications" (Solicitudes) cerca de la parte superior de la pantalla.
   3. Seleccione "Add Application" (Agregar solicitud).
   4. Complete y envíe la solicitud.
Nota importante: Si tiene varios estudiantes, no puede enviar una solicitud mientras se encuentra en la vista "All Students" (Todos los estudiantes). Debe seleccionar a un estudiante individual antes de agregar una solicitud. No obstante, la solicitud se aplicará a todos los miembros de su hogar, por lo que solo necesita enviar una solicitud por hogar.
