HIGHLAND ATHLETIC INFORMATION
The Fall Sports Meeting for high school athletes was held on Thursday, August 10 at 7:00 pm in the High School Commons. Mike Lewis, District Athletic Director, explained the WIAA, district, and school eligibility requirements. Coaches explained their team rules and expectations. If you have any questions please contact your athlete’s coach.
The following items must be completed and turned into the high school office (grades 9-12) or the junior high office (grades 7-8):
_______Parent Permission Form (signed and dated)
_______Insurance (Parent Permission form)
_______School insurance – application/fee turned in to office
_______Private insurance – name of company and policy number on parent permission form
_______Shared Responsibility for Sport Safety form signed
_______Physical signed by a doctor (within past 24 months)
_______Emergency medical release form signed and dated
_______Signed student Athletic/Activity Code (parent and student) (Parent Permission form)
_______ASB Card
$30 – High School
$15 – Junior High (Athletic Fee)
All forms and fees must be in the office before the athlete can participate in practice. Do not turn in forms to coaches. The athlete must be cleared by the athletic department before practicing.
High School : Tuesday, Wednesday, Thursday from 8:00 - 11:30 am.
